Local systems should agree a plan to improve outcomes across the Health and Wellbeing Board area for improving the proportion of people discharged home using data on discharge to their usual place of residence. The aim is to monitor ‘home first’ principles and inform service planning.


This measure is the total number of discharges during the month following a stay in an acute bed where the patient is aged 18 and over expressed per 100,000 population aged 18 and over. The data excludes those who have died in hospital, regular day/night attenders, and those with a zero length of stay.


This data is not currently collected at a local authority footprint in national reporting, so has been derived by the Better Care Fund team using information recorded through hospital patient administration systems, available through the Secondary Uses Service (SUS) database. This enables identification of the local authority footprint associated with the patient’s usual place of residence.
